There was a technical issue that required me to go in and make one tiny modification around the time you had that problem. As a result, anyone taking the survey during that window of time was unfortunately booted out. The problem is fixed and you should have no trouble now.
This was a pharmacological treatment survey. Including non-pharmacological to a comparable level of detail would have required much more work and time, and I wanted to release the survey sooner rather than later. I hope to do a non-pharmacological survey at a later date.
